Out Door Education:

Our 5th graders will be going to Our Door Education from May 6-10, 2013. Please

make sure you have all information into the school office by April 26, 2013. All medication forms

must sign by parent and physician. Include the dosage and frequency, specified by the physician.

We want all of our children to have an unforgettable experience. If you have any questions please

call Clara from 8-3 Monday through Friday.



HEALTH OFFICE INFORMATION:

Medication

Students are not permitted to possess or administer medication by themselves. All mediation,

including aspirin, ibuprofen, etc. must be brought into the health office by an adult with written

verification from the student's physician as to the name of the medication, the proper dosage and

the means by which it is to be administered. ALL medication must be kept and administered in the

Health Office.

Crutches, Casts and Braces

If your child needs to use crutches, or to wear a cast or brace, please have a doctor's note giving

permission to attend school. District policy requires them to stay in the office or the library during

recess and lunch. They may NOT be on the playground.


Illness or Injury at School

If a child becomes ill at school, parents will be notified immediately. If parents cannot be reached,

another adult designated on the student's emergency card will be asked to pick up the child.

Insurance

Accidents which occur at school are not the responsibility of the school. Insurance forms are made

available for your convenience at the beginning of the year.
